At registration on the day of surgery, you will be required to pay the anticipated patient balance (deductible, copay, and coinsurance). This payment is for the Southwest Surgical Center (Sixty Fourth Street LLC) facility bill only.  Services performed at the facility by your surgeon, anesthesiologist, and other providers involved with your care are billed separately. Additional patient balances may apply to their claims.

If there is no insurance coverage, payment for the Southwest Surgical Center facility services is required on or before the date of surgery. Special consideration may be given in cases of financial hardship. Our Business Office employees can assist you in the completion of the required forms to qualify.
